Community Place Hub

Rooted in community and collaboration, the Community Place Hub strives to improve the health, well-being, quality of life, and personal development of community members of all ages in York-South Weston. This is achieved by creating a welcoming and inclusive space where, in collaboration with a local service provider network (SPN), we offer a variety of barrier-free programs and services.

The Community Place Hub offers:

  • Health Access: improving health service accessibility by assisting people to navigate the health system. The Health Access Model promotes a collaborative, client-centered approach, greater accountability and outcomes, making a substantial difference in the lives of community Members.

  • Drop-in Program: provides free daily services including information and referrals, affordable lunches, printing, copying, faxing, internet, and social and recreational activities.

  • Peer Support: Weekly peer-to-peer program provides space for people to connect with others who share the same experiences.

  • Wellness Fairs connect community members with local health and social care supports and providers.

  • Mental and physical health & wellness programs

  • The Weston Sewing Hub at the CPH, initiated in 2017, is part of the City of Toronto's Community Reduce and Re-use Program, and is one of the first of its kind. Program activities include regular instruction on sewing, alterations, and repairs for basic, intermediate, and advanced levels. The program also encourages repurposing, which helps people develop skills and save money by designing and making their clothes from rescued fabrics. This project contributes to the development of the Textile Collection and Reuse Strategy.
